Old Saburo got so butthurt over Morgan, Johnny and the rest of the crew blowing up his precious tower that he soulkilled Johnny and put him into the engram purgatory where he can torture Johnny for eternity. I suppose it was his personal revenge, especially since Saburo had a god complex.

The real story ends when Smasher kills Johnny after he frees Alt. Everything after the first fade to black inside the tower is made up, except Murphy’s voice. She would have been the last person he heard before dying as she managed to sneak back into the tower to inject Johnny with Soulkiller so that he may have a chance to live again sometime in the future. Smasher fricked off to the roof to fight Blackhand, that’s how Spyder managed to get to him

Also when I read the wikia and saw that Morgan Blackhand was supposed to be there and the entire thing bankrolled by Millitech but Mike Pondsmith told CDPR no about including Blackhand
Pondsmith said he did this because he has a role for Blackhand to play. He also said that Johnny's memories are a vague recollection, his memories were damaged by rad poisoning, the condition his body was and imperfect engram recording. I think his John Wick one man army fantasy is a way for his ego to cope with the fact that he did almost nothing before getting btfo'd by Smasher instantly. He's a known narcissist after all, dude has a terminal stage main character syndrome. Hell, he thought at first that Arasaka kidnapped Alt to get to him, a literal fricking glorified bard and was never interested in Alt enough to actually find out that she was actually a generational talent genius netrunner. Always making everything about himself.
