When it comes to main characters it's usually rare for an important character in a piece of media that usually has some kind of conflict or combat or something to be pregnant. On top of all of that they'd need new animations to prevent clipping. This is basically "why aren't there more fat characters in games" but to a further extreme.

It's not a challenge, it's an expense, and one that doesn't wow people in trailers or make the experience smoother so it's not generally done.
