Uncommon for a tape to get eaten from the VCR malfunctioning. It's not super rare in very late cheap models like those 10" VCR/TV combos but most cases I would bet were done from someone, probably a kid, trying to pull the tape out without ejecting it.

Never happened to me and i used tons of vhs recorders and players. I lost a lot of audio tapes to all sorts of players though.
