IIRC in FES the scene plays out in the morgue her body starts showing signs of life, meaning her Persona rebooted her back to life as part of her abilities. I don't know how it's handled in Reload, but it's very easy to miss in FES because you have to talk to Junpei on a bunch of random days and tell him to go visit her in the hospital. There's nothing to indicate when you need to talk to him, and if you miss one day she dies.

Nope. After Junpei balks on buying Chidori flowers, IF you buy the flowers yourself later and give them to Junpei, he gives the flowers to Chidori which is where she stores part of her soul leading to the revival event.
IF you don't buy the flowers, she remains dead.
