Your vault was invaded by Chinese spies before it sealed, they were immediately killed but created sleeper agents in the form of the cryogenically frozen vault dwellers to sabotage any chance America had of rebuilding. One day, you uncover the spies' graves, and accidentally play the trigger code. Everyone but you goes insane, trying to kill you, and you're forced to leave the vault. On the way out, you learn the spies had a base nearby in Chicago, on the previous site of an CIA project called MK ULTRA. You learn there may be a way to cure your vault before they escape and kill innocents, or themselves. It's a race to fix them before the worst happens.
The twist is, that you are the only sleeper agent, and an unreliable narrator who went insane on hearing the activation phrase, and now is working to sabotage the slowly rebuilding America.
Instead of it being about a Father looking for his Son, or a Son looking for his Father. I would make it a Father x Son boyfriends duo. So neither of them have to look for each other and instead they have to survive together in the apocalyptic world, having each other's backs, passionately kissing, and making mad incestuous love.

And all the other islands rely on shit getting shipped to them from Honolulu, which has to get all its shit shipped from California. I just don't think there would be anyone left there.

Alright, in that case I suggest you look into them along with the history of the Mardi Gras Indians. The New Orleans Mardi Gras organizations are descended from pretty old secret societies that put on debutante balls and things like that. Some point in the 90's, the government said you can't parade if you are racist, so the oldest and most respected organization, Comus, stopped parading. To this day the other old and respected organization, Rex, pays tribute to Comus in their own tableau. And the Mardi Gras Indians are also a secret society that in ye olden days didn't just dance at each other, but actually were engaging in physical fights to settle old grudges and scores while dressed like injuns. Both these groups are incredibly secretive, and both consist of powerful members of the white community and black community respectively.

Plot impetus is that Texas has started getting its shit together and is starting to expand. Choose to either side with the Texans, or defend an independent New Orleans

Had an NCR-like functioning government and active civilization for many years but got wiped out by something, we are able to put together what happened by exploring the layers of civilization and seeing the things they'd built. There are obvious eras of development, crises, and conquests on par with west coast history told by lore and environmental storytelling going back to prewar
In the end you avenge them and restart their nation

The Baja California. The remnants of the old Cartels have reformed, forming a sort of confederation of states with loose central authority, they are a sort of pirate republic. A group of technoconquistadors from South America, being from a state that wasn't hit as hard as the rest of the planet. The NCR, because you got to have them. The big major gameplay addition is a Sid Meier's pirates style area in the water around the Baja. You can land in a city, step out with a group of your pirate bros, you can raid NCR shipping or the technoconquistadors. The whole idea would be to recreate the whole "scramble for the Carribean" feel. Where there are a bunch of mostly independently governed cities that exist solely off cash crops and crap like that. Mexico feel on one side with the cartel, aristocratic europe feel with the technoconquistadors, pirate feel in the ocean, and American-puppet banana republic on the NCR side.
I really liked the Idea of Van Burens plotline so I'd probably do something similar to that, but post NV rather than pre. I know for sure I'm a worse writter than Emil so some of this will sound absolutely moronic. You start out held hostage in a Vault turned prison by NCR forces as they struggle with resources after the second battle of Hoover dam and Vegas. The cause is unknown and can be determined by the player (I know, real TES hours) Eventually you're able to break away from the facility as since manpower has not only been low but so has morale, the vault is mostly regulated by dumb bots. As you explore the wasteland once you've broken free you eventually start seeing thay the land is oddly sick. The once thriving nation of NCR has been having multiple symtoms that align with a pre-war bioweapon called the blue flu. Something that remains odd about the virus though is that the last generation of mutants have been unaffected likely due to FEV. As the few remaining non-sick NCR viruses begin looking into the flu the only possible solution to inocculate and save the wasteland is FEV Vats. You as the player then have the choice to either develop a cure for certain factions, can modify the FEV to be like Culing-13 and fully genocide the wasteland, or begin the process of creating more mutants to populate the wastes again. And this has to be done before you and the scientists die from the flu that you were unknowingly spreading from the prison.
