She's from the Nechronica lore/setting book. Some anon scanned parts of it ages and ages ago but I don't believe it's ever been properly catalogued or translated. This is the only other piece I have saved from it.
The only times I have died is if the GM was being a complete shitter or the one time I knowingly was walking into a trap and did it anyway in a game of chicken.
