I don't remember any evidence of anything clearly problematic that miquella has done. I'm guessing that a major aspect of his character will be the question of the morality of compelling someone to do something arguably good or positive, but that they wouldn't have otherwise done. Miquella has seemingly given many groups and individuals genuine reasons to admire and respect him, so this concept of "compelling" someone is unclear. He understands and manipulates wants and needs, but there's some other element to it, as the bewitching branch suggests
No, the expansion has the ultimate soulsborne twist. There is no Miquella boss fight, the Lands Between is healed, and then all the remembrances of the characters show up and start clapping.

He may or may not be the final boss. He's just a spirit without a body at this point, same as Melina and Ranni, and you don't fight either of them.
