Game is 3D and has an X, Y, and Z but cheats several calculations so that they only function in 2D. Most important things like player, monsters, and projectiles are fully 3D although monsters have infinite height to reduce complexity of player being directly above or below.
Added to this. Wolfenstein is 100% 2D. There is no height, everything is on the same plane as the player. Projectiles and monsters do not have height. The game could be a top down shooter and would function identically other than the perspective shift.

Don't care about sectors or lack of room over room. The final image and perception are the only things that matter.

The "infinite height" only applies for collisions with the player. All enemies have a 3D hitbox and a cyberdemon won't fit in a room that's too low for him. Doom removed as many calculations as possible so the game could run on pretty much everything. Another example of tjis is when you use a hitscan weapon, the game only calculates hit detection using the hitbox's 2 diagonals instead of 4 sides.

But everything in game still has a standard 3D cube bounding box for regular collision. So an imp can walk under a doorway that's too short for a cyberdemon.
