Looks good, but my fear is that all the planets are just whole lot of empty with some Skyrim tier "cities" sprinkled in between.
They confirmed three big cities on separate planets. We haven't been shown smaller settlements. Can also be taken as confirm how a frickton majority of the planets will just be empty, unsettled outpost playground for "le emergent gameplay". That "thousand planets" Todd ended with isn't really giving much to look forward to.
From everything I've gathered, you're almost on the money.
Todd has said that they put a whole lot of empty iceballs and such into the game just because they could, to add more realism and empty space to the game about exploring the vastness of space. You can land on these places, but much like a researcher or explorer out in the arctic, you're going to have to find your own enjoyment.
There'll be four "major cities", the three we've seen so far appear to be larger than Skyrim/Fallout ones. A couple crowd shots show 30-40+ NPCs in a single part of the large cities, which is more than half of the largest city in Skyrim. They've claimed New Atlantis will be the largest city they've ever created. Akila City on the other hand looks only a smidge bigger than Whiterun.
If there are Skyrim-tier settlements outside the four cities, they'll be the Starfield equivalent of Morthal and such.

Whether it's through the game itself or mods, I know there will eventually be a way to space people. It just has to happen.
The main question for mods is whether in the base game you'll be able to do spacewalks from your ship, and whether NPCs or the player can suffocate. If yes to both, then it would take a very simple mod turning surrendered enemies (Social perk in the base game can force an enemy to stop fighting for a time, expand on that a smidge) into pseudo followers to do this:

I'm curious if the game actually lets you walk about your spacecraft mid flight or you can park up somewhere in the void. I'm all for planet exploring but if you are able to indefinitely live out in space that would make for some neat playthroughs. The mods alone will speak for themselves in time.

Largely, yes. I think the difference in simulations vs rpgs is depth vs breadth. With a good simulation you can do all kinds of things and create all kinds of scenarios, but many stories don't perform well in them unless you create the story yourself. RPGs won't go as deep as simulations but they can go 'further' in a sense because the game is handling the systems that a player would manage in a simulation.
Like creating a Minecraft adventure game from scratch vs playing one someone else made, and also the difference between playing Mass Effect vs reading the event text box in Stellaris. It's both the amount of work the player wants to put in, and the difference in the scale of events.
That's exactly what it will be, you travel to "planets" via loading screen and it will load a pre-generated landscape where you will walk around and zap resources or whatever.

Me, I love seeing all the explanations and explorations of how technology could evolve over the centuries and how it may affect people's lives. The first season of The Expanse was great for that. Guy gets his arm chopped off by an industrial accident, the next scene he's receiving medical treatment to seal the wound quickly and talking with his co-worker about health insurance; whether the company is going to frick him with a second-rate prosthetic that doesn't even have temperature sense.

This is huge. Fallout 4 had to be moronic strict out of the box about custom animations because of the Havok license expiring. Bethesda having their own in-house animation system means it can be included in the CK, so custom animations for weapons, characters and other shit can be done from day one by anybody.

What's the over/under on the game permitting us to play as a pirate? My presumption is either 0% or only in the sense that the lack of actual space travel mechanics means you'll never encounter another ship naturally.
I'd put it at 80% currently. You can join the Crimson Raiders, the pirate faction, and while I'm also on the same assumption that coming across ships won't quite be as natural as flying along and spotting one, (I think you can warp into orbit to meet ships based on if there's a ship icon over the planet, like in the bottom left of pic related) Todd has mentioned in the IGN interview that space encounters will have some variety:

No, I bought it up because I know the fps limit and unadjusable for are two common complains with their games, and both were addressed years ago in their most recent title. That's all.
